These evaluations are for my reference only. They will be kept confidential. I will use them to evaluate how well each group is functioning and to assess whether full scores are merited for each group member. If these evaluations indicate that there are problems within your group, I will discuss this with you. I reserve the right to require any individual to submit individual reports rather than group work, and/or to give individual grades that are lower than the group grade, if the situation appears to warrant such action.
Your comments here should relate only to the functioning of your group on this particular group project. They are not an evaluation of your group members as people. Please make positive comments about your colleagues as well as offering constructive criticism. I refer back to these comments when writing letters of recommendation for students, and they have proven to be very helpful.

6. Please rate the contributions of each group member to this particular group report. Use a scale from 0 - 100%. (If you felt, for example, that all three group members contributed fully and equitably, you would give each group member a rating of 100%.)

Please sign and date your evaluation, then fold it in half and staple it shut. When you turn in your group report, attach an evaluation from each group member. Your report will not be considered complete (and thus counted as on time) until all of the evaluations have been submitted.
